The little red finger is pointing to the area of this exhaust valve that has burned away. The valve opens by pushing out of the combustion chamber (that's what you're looking at). This condition prevents this particular cylinder from producing any power and causes an incredible emissions problem.

Regular valve clearance adjustments can help prevent this costly repair. Adjustments are usually performed during Intermediate or Major services.
